By completing this survey, you will be automatically entered into a prize draw to win one of four (4) $50 Amazon gift vouchers.

Eligibility

• The prize draw is open only to recipients of this survey invitation.

• Participants must complete the survey in full to be eligible.

• Only one entry per person is permitted.

Prize

• Four (4) winners will each receive a $50 Amazon gift voucher.

• Prizes are non-transferable and cannot be exchanged for cash or an alternative reward.

Winner Selection

• Winners will be selected at random from all eligible survey responses received.

• The draw will take place after the survey has closed.
